Remedi Medical Aid Scheme has introduced the Disease Prevention Programme to support members who are at risk of diabetes or cardiovascular disease. If you meet the eligibility criteria, once enrolled in the programme, you will have access for 12 months to dedicated clinical care, coaching support and additional Scheme benefits to help improve your health.
Introducing the Disease Prevention Programme
This programme, together with your Premier Plus GP and supported by your Health Coach and a Dietitian within our network, will help you actively manage your risk of developing a chronic condition. The programme also provides additional benefits to help you monitor and manage your health, ensuring you receive high-quality coordinated healthcare and improved health outcomes.

How to join the Disease Prevention Programme?
We use your Health Check results, health claim patterns, family history and other relevant information to determine whether you are at risk of developing diabetes or cardiovascular disease. If you meet the Scheme's eligibility criteria for the programme:
- A Health Coach will get in touch with you to explain how the programme works.
- A Premier Plus GP can enrol you in the Disease Prevention Programme through HealthID, once you have given your consent.

What is a Health Check?
A Health Check is a simple but helpful set of basic health screenings and preventive tests which you are covered for annually. These include:
Blood pressure
Blood sugar
Cholesterol or lipogram tests
Weight or body mass index (BMI) assessments
Non-smokers declaration (NSD)

- To better understand the health status of yourself and your family, where applicable.
- To enable early detection of potential health risks, allowing for earlier treatment and better management of more serious conditions, and potentially identifying you as eligible for the Disease Prevention Programme
- To help reduce your healthcare costs over time
- To assist your doctor and other healthcare providers in delivering more personalised and appropriate care

The programme uses an advanced predictive model to identify eligible members. This assessment is based on your Health Check results. Members whose assessment results indicate a high risk of developing diabetes or cardiovascular disease may be eligible for the programme. From here, your Premier Plus GP may enrol eligible members in the programme.
The goal of the programme is to reduce your risk of developing diabetes. This may include addressing high blood glucose levels through a healthy eating plan, regular exercise and where applicable, appropriate medication. During or at the end of the programme, your Premier Plus GP will guide you on the next steps to take. These may include continuing with the dietary and exercise changes you have made or continuing with the prescribed medication. Alternatively, you may be diagnosed with diabetes.
Your Premier Plus GP will help register you for diabetes on the Chronic Illness Benefit (CIB), which will provide you with access to benefits to help manage your condition. You can then enrol in the Diabetes Care Programme to receive additional support for managing your diabetes. You can learn more about these programmes here.
